This article explains how the mutation forms can be set. So that employees (with an employee login) can submit mutation forms.
Create mutation form model at master level
To be able to use the employees for mutation forms, a 'Mutation form model' must first be created at the highest level (debtor level). The model can be created at Settings & Global settings & Mutation forms settings. The image below shows where you can find these settings:
Then you see a standard model here called Mutation Forms Model (Default). In this model everything is arranged, and the settings can be adjusted as desired (see next section for more explanation). It is also possible to create an entirely new model. You do this by clicking on 'New model', see image below:
Here you only need to choose a name for the model and then click on 'Save'. When this new model is created, you can adjust the assessment chain on the 'Maintain model' tab and add or delete mutation forms.

When you choose 'Model maintenance' you can select for which model you want to make adjustments:
Employee mutation forms (assessment chain)
In the upper part of the model you can set up the assessment chain of the employee for mutation forms. Every request for mutation submitted by an employee ends up in the assessment chain. The image below shows an example of an assessment chain for the employee mutation form, which is divided into four different steps:
In this example you can see the following:
Step 1 - Employee requests a change via a mutation form
Step 2 - The form ends up with the manager. In this case the department manager, if the employee does not have a department manager, the form will be sent to the employee manager. If it is not there, the form will be placed in the 'Unallocated requests' box.
Step 3 - If the manager has approved the form, the form will go to a specific user (with a company login), who can definitively approve the form so that the changes in the system are implemented. The specific user can be set at company level, if no specific user is set at company level, the form will be sent to the company login that is active at the company.
It is possible to remove users from this chain. You do this by clicking on the relevant block and unchecking the checkbox. Of course, a user (manager or customer) must always be active in order to be able to approve the employee's transaction form. This step will then be skipped in the assessment chain. The image below shows which check mark should be turned off:
It is also possible to change the users in the chain. When you - as shown in the image below - want to have the employee manager above and then the department manager. You can do this by clicking on the user and dragging up or down:
Employee mutation forms (forms)
In addition to the assessment chain, the forms can also be selected here. You do this by choosing 'Select new employee form':
You can then add or remove forms to the model by turning the check mark on or off:

In the models for employee forms, we offer the option to create multiple assessment chains. This makes it possible to have certain employee forms run via a specific assessment chain.
1. Select the prefered model mutation form.
2. Add a new assessment chain.
3. Adjust the chain as perfered by clicking on the block.
4. Select the desired employee forms for this assessment chain.
Please note!
It is not possible to choose the same employee forms for multiple assessment chains. When a certain form is assigned to a chain and is then selected at another chain, the form will be moved from the chain where it was first included to the chain where the form was last selected.

For the mutation form 'Contact' & 'Contract', the manager can be set as final approval. In short, if the assessment chain is set up correctly (deactivate company approval), the manager can approve and apply these forms.
Set up mutation form model at company level
A mutation form model can be selected at company level; you indicate that the settings in this model apply to this company. You do this at company level in Settings & HR Settings - & Mutation forms settings. Then choose 'Edit' and here you can choose the desired model. The picture below shows this:
You can also set the specific users at company level, you can do this by clicking on the relevant block and selecting the user in the drop down menu:
Set the wage model for the declaration form
If you have indicated on the employee or company mutation form that the 'Declaration form' is available, you can select a wage model at company level that contains the wage codes that can be chosen by the employees or employer in the declaration form. The figure below shows that the standard declaration model is used:
